                                              The application process is slightly different for each situation depending on type of dog requested.  But the usual process includes:

                                               * Contact Dot Hyde-Williams at A.C.T.S. to request an application packet (packet customized to each situation) either by phone (603-569-9991) or by email (hydewilliams@assistancecanine.org)
                                                    
                                              * Each packet generally includes:
                                                             * Letter explaining A.C.T.S. application and follow-
                                                               up procedures
                                                             * Request a Dog Application
                                                            * Letter from medical professional certifying the need for a
                                                               service/assistance dog
                                                            * 2 Letters of reference

                                                   
                                              * Application should be returned to A.C.T.S. along with a nonrefundable $25 application fee

                                                    *  It takes about two months to get the completed application back.    Once returned, an A.C.T.S. representative (usually Dot) will contact you to set up an in-home appointment to get an idea of the living situation and what tasks the dog needs to perform to meet your needs.
